As a new patient, you will be asked to complete registration and medical history forms. It is important for you to provide all requested information. The demographic and medical history forms can be downloaded and prepared in advance of your appointment.

- Please bring any pertinent medical records with you to your appointment. If your records include any x-rays, CT scans, or MRI’s, please bring both the films and written report with you.
- Please bring a list of your current medications, strength, and dosing directions.
- Please bring your insurance card(s) to your appointment.
